Our Services
Our team of registered nurses provides you with quality care. They work closely with you, your physician, family, and caregiver in order to keep you safe and help prevent future injuries or illness. Nursing services include:
- Conducting a complete assessment of your current condition
- Developing and implementing an individualized care plan
- Administering treatment and medications
- Working with your pharmacy on any medication changes
- Updating your physician about health changes and progress
- Directing other members of the home care team (e.g., Licensed practical nurses, home health aides, and personal care assistants.)
- Cleaning, observing, and dressing wounds
- Teaching basic tasks of care to family members
Our RNs will also help arrange for any needed equipment and supplies.
Alisam Care, LLC provide a variety of personal care services, sometimes referred to as “hands-on” services. These services include helping you with:
- Bathing, washing hair and shaving
- Dressing and toileting
- Feeding and meal preparation
- Mobility and assisting with range-of-motion exercises
Alisam Care, LLC also are responsible for:
- Performing delegated tasks supervised by a registered nurse
- Documenting care and observations, notifying the registered nurse of any health changes
- Following and assisting you in your plan of care
PCA (Personal Care Assistant) services may be available to clients who are enrolled in Medical Assistance, a MCO (managed care organization), Minnesota Care expanded benefits, (AC) Alternative Care, or a waiver program. Our Personal Care Assistants provide supportive care, helping you live as independently as possible.
PCAs provide services and support to help you with:
- Activities of daily living (ADLs), such as bathing, grooming, and getting in and out of bed.
- Health related procedures and tasks, such as assistance with self-administered medications.
- Observation and redirection of behaviors, to keep you safe at home.
- Instrumental activities of daily living (IADLs), such as housekeeping, shopping, and meal preparation.
Our social workers help clients and families with a variety of issues, including:
- Finding resources for necessary care
- Coordinating meal delivery and other necessary services
- Advising on housing, transportation, and long-term care options
- Helping with outreach services, crisis prevention, and social rehabilitation
- Planning supportive services for your return home
- Assisting you, your caregivers, and other homecare staff
Alisam Care LLC social workers evaluate your physical and mental needs, as well as your support system. They are accustomed to handle challenging situations; whether someone is in need of service and resource recommendations, or looking for agencies to help cover medication and service costs. Our social workers use their expertise to provide assistance when it is needed most.
